Long Term Health and Wellbeing Programmes are free, 2½ hour, group based 6 week programmes for adults with long term physical or mental Health conditions and for adults who are caregivers to family members, friends and neighbours, living with long term physical or mental health conditions or disabilities.
Courses are run throughout the year in community settings and on zoom.
By taking part in our programmes you will:
Understand your symptoms and their impact on your health condition
Learn new skills to help you manage your health condition and improve your quality of life.
Develop confidence in the daily management of your specific health condition.
Meet others, share experiences and make new supportive friendships.
Learn how to develop more effective relationships with your healthcare professionals.
Plan for the future.
We offer the following programmes :
Long Term Conditions - For adults who have physical or mental health conditions
Diabetes Self Management Programme - For adults living with Type 2 Diabetes
Cancer, Thriving and Surviving - For adults who have cancer or are survivors and fear a recurrence
Caring for Me and You - For adults caring for a family member or friend living with a health condition including Dementia carers.

To access Long Term Health and Wellbeing Programmes, you must meet ONE of the following eligibility requirements: Live or work in the Dudley Borough Be registered with a Dudley GP Care for a family/friend in Dudley.
