Dudley Brain Injury Service and Disability Support Team, provides case management support and information to people with brain injury, their family and carers.
The Disability Support Team also provide case management support for patients who have complex long term conditions and neurological conditions.
The service is available to people who are registered with a Dudley GP or who live within Dudley borough and are aged between 16 and 67 years.
