Dementia and Frailty Link Worker

Everybody forgets things from time to time. But if you are noticing problems with your memory, or that of someone close to you, it’s important to get some help and advice.

If you belong to a GP practice listed below*, then you can ask reception staff for a referral to the Link Workers – Lisa Harland and Stephanie Powell. Alternatively you can self refer by emailing: dihc.halesowenmemory@nhs.net

They can visit you and help with:

• Identifying areas where support is required via a holistic support plan. 

• Referring to services for Social Care/OT and Carers’ Assessments, Carers support, Telecare, Benefits and Legal Advice, housing issues, healthy lifestyles, and emotional support, befriending and community activities.

If appropriate, we then liaise with GP's and refer into the Memory Assessment service with the aim of getting a timely diagnosis and further support.
