Anyone worried about Dementia, diagnosed with the condition, their families or carers can ask for help from a Dementia Advisor.
The offer tailored information and advice about living well with dementia. They can signpost and connect people diagnosed with Dementia and their carers to a range of services suited to individual needs and requirements.
This can include:
Help with a Diagnosis
Dementia information and advice
Carer support
Independent living advice and signposting
Community and social support
How to access care and support.
You can contact the Dementia Advisor service by calling 01384 816039 or emailing demgate.services@dudley.gov.uk. for a referral to be created for the team
Dementia advisors are also present in The Crystal Gateway on Monday, Tuesday, Thursday and Friday for face to face walk in advice.
You an also come along to a Dementia Information Session held monthly at The Crystal Gateway Dementia Hub, Brettell Lane, Amblecote, DY8 4BN. This 2 hour session is to share information about living well with dementia. Call Crystal on 01384 813315 for dates.
