Self Management Programmes are free, 2½ hour, group based 6 week programmes for adults with long term physical or mental Health conditions and for adults who are caregivers to family members, friends and neighbours, living with long term physical or mental health conditions or disabilities.

Courses are run throughout the year in community settings and on zoom.

Self Management courses help you understand your symptoms, learn tools and techniques to help you take more control of your health and well-being.

Carer courses, facilitate skills to help you to better manage caring responsibilities, as well as take time to manage your own health and well-being on a day to day basis.

Diabetes Self Management Programmes help you to understand and manage your type 2 diabetes, learn tools and techniques to help you take more control of your health and well-being.

Eligibility Notes:

To access Dudley Self Management Programmes, you must meet one of the following eligibility requirements

  • Registered with a Dudley GP,
  • Live in Dudley,
  • Care for a family/friend in Dudley or
  • Work in Dudley

Referral Notes:

Referrals can be made via Health care professionals, Social prescribers and Job Centres. You can also self refer by sending an email to smp@dudleycabx.org or text 07976 637404
